GENERAL COMMENTS

During most of my clinical career one quarter of my clinical time was spent caring for newborn babies and following up graduates of the Special Care Baby Unit. Many of these babies were at particular risk of disability because of their experiences during the perinatal period.

Early on, however, neonatal care took much more time; the neonatal and infant death rates in Hawkes Bay were higher than in other places in NZ. It seemed likely that this was due to a lack of good obstetric care and skill in assisting distressed new-borns.

Within a few months of my tenure, another possible explanation presented itself; a very high rate of serious and often fatal infections in Maori new-borns was noted. Most of these babies were well at birth, fed and thrived for the first few days of life and became ill suddenly. Apart from being Maori, the only other striking feature of their care was that they received injections of iron, soon after birth.

This treatment was given to prevent iron deficiency, a serious problem in Maori infants and was very effective in this. Unfortunately as our study over several years showed, the iron injections promoted serious infections in Maori new-borns, and these disappeared when the iron injections stopped. Our studies and research by others convinced us that iron injections in early life could result in serious infections in any baby and should be abandoned. Our concerns, and the evidence behind them were sufficient to have a paper published in the journal “Pediatrics”, the official journal of the American Academy of Pediatrics. This paper and shorter reports on iron and infection from 1973-91 are listed above.

It is now accepted that anything that makes our body iron more available to invading organisms, promotes infection.

New-born’s apart my clinical work was in the form of ward rounds and outpatient clinics dealing with a wide range of medical paediatrics in other age groups.

I have a particular interest in respiratory diseases in children and used to hold regular clinics for cystic fibrosis and asthma in the Hawke’s Bay area.

Over the years I have worked in conjunction with colleagues in Community Health, particularly in the areas of cot death prevention, better community asthma management and the treatment and follow-up of children with Tuberculosis and Rheumatic fever.

I was Chairman and Liaison Paediatrician of the Mortality Review Committee (LMRC) the Hawke’s Bay area. This Committee’s purpose is to learn lessons about the ways in which deaths can be prevented in children and young people aged one month to 24 years. The LMRC gathers data about local deaths and enters this into the National database. Through this database information from all parts of the country will change local snapshots into a national collage which will inform the Government on ways to reduce preventable deaths.

The nature of general paediatrics in a provincial hospital means that professional collaboration takes place with many other specialist colleagues such as those in obstetrics, surgery, orthopaedics, intensive care, anaesthesia, pathology, radiology, ophthalmology and dentistry. The fairly frequent need to use tertiary services in areas such as cardiothoracic surgery, neonatal surgery, paediatric nephrology and rare metabolic diseases give the general paediatrician a unique opportunity to observe the way in which the health system operates regionally and nationally.

I have had the opportunity of serving on and chairing a number of committees in the broad areas of health. Some of these have been to do with professional activities (New Zealand Medical Association), some to do with the Postgraduate Medical Education (Specialty Advisory Committee

in Paediatrics, New Zealand Council for Medical Education, Hawke’s Bay Postgraduate Medical Society) and some to do with community groups (Hawke’s Bay Asthma Society).

I have had experience as a Board member with the Asthma and Respiratory Foundation of New Zealand,  the Central Regional Health Authority, the HBDHB and as a member of the Council of the Hawke’s Bay Medical Research Foundation.

I have had the opportunity to collaborate with colleagues from major centres in research related to Cot Death and Asthma. I have a continuing interest in iron and its relationship to infection, and in obesity in children.

Following retirement from full time practice in General Paediatrics (Hawkes Bay Health Board) in June 2006 I worked as a locum Consultant in Paediatrics until 2013

From July 2007 I spent about three months in General Specialist locum Hospital Paediatric work in various hospitals (Blenheim, Timaru, Southland, Masterton) and several months doing Specialist Paediatrics at Timaru Hospital (until November 2008). I worked as a locum consultant Paediatrician,  Hawkes Bay DHB from February to August 2009. I completed a short term contract doing paediatric consultant out-patient clinics Hawkes Bay DHB late 2010.

I was elected to HBDHB for another term starting December 2010 and retired in 2013
