I am writing to acknowledge receipt of your letter of 26 July, 1978, in which you asked that Mrs Audrey [Audree] Curtis be recommended for an award From Her Majesty The Queen.

Before I submit her name to the Prime Minister, I wonder if you would be good enough to let me have her full name and residential address.

Incidentally, I enclose a copy of an instruction leaflet which sets out the details which are required when nominations are being considered. [not included]
